The St. Francis Center for Surgical Care is taking appointments. We take appointments directly from the patient; however, if your insurance requires pre-authorization prior to treatment it is your responsibility to obtain this from your primary care, or referring physician.
Patients are seen by appointment only, and we respectfully request that you notify us well in advance if you are unable to keep an appointment. This will enable us to give this appointment time to another patient.
Your insurance information and demographic information are requested when your appointment is made, so that we may verify your coverage, deductibles and policy guidelines. Failure to provide this information will deem you a self pay patient and payment will be due at the time of service.
Our fees are based on usual customary and reasonable charges based on a conversion factor used for our geographic area.
We accept Visa, MasterCard and Discover.
Our office hours are Monday thru Thursday 8:30 a.m. to 4:30 p.m. and
Friday from 8:30 a.m. to 2:30 p.m.
There is a physician on call 24 hours a day, seven days a week. Please call 706-324-3243, and the answering service will contact the physician on call.
Your Role and Responsibility as a Patient
If your insurance requires that you have a referral from your primary care or referring physician, it is your responsibility to secure this. If you come for an appointment without it and are seen you will be deemed as self pay. You are responsible for deductibles, co-pays and non-covered charges pre-operatively. Should you be uninsured, we will make every effort to work out arrangements suitable to you and your physician.

- Evergreen Medical Group and Health Plan

- Medicare and Railroad Medicare
- Georgia Medicaid ONLY
- Most other indemnity and major group plans
- Workers' compensation upon approval
